All-on-4 Dental Implants in London

A full set of fixed teeth held on just four implants per jaw. Planned and placed by Dr Sahil Patel at our Harley Street clinic.

What Are All-on-4 Dental Implants?

All-on-4 is a way of replacing all the teeth in one jaw with a fixed bridge held on four dental implants. The two front implants are placed upright and the two back implants are angled, which lets them use the stronger bone at the front of the jaw and avoid the sinuses and nerves further back. All-on-4 is one of several options covered on our Dental Implants London page.

Because the back implants are angled, many people who have been told they do not have enough bone for implants can have All-on-4 without bone grafting. The bridge usually carries 10 to 12 teeth and is screwed to the implants, so it stays in place and does not come out at night.

Who Is All-on-4 For?

  • People who have lost most or all of their teeth in one or both jaws
  • People with several failing teeth who want to replace them all at once
  • Denture wearers who want fixed teeth that do not slip or cover the roof of the mouth
  • People with some bone loss who want to avoid or reduce bone grafting

All-on-4 may not be right if you have very little bone even at the front of the jaw, a very strong bite or grinding habit, or uncontrolled health conditions. In these cases, All-on-6 dental implants or other options may suit you better.

Benefits of All-on-4

  • Fixed teeth: your new teeth are screwed in place and do not come out at home
  • Fewer implants: four implants per jaw instead of one for every tooth
  • Often no bone graft: angled implants make the most of the bone you have
  • Teeth on the day, where suitable: fixed temporary teeth can often be fitted on the day of surgery
  • No palate: unlike a denture, the roof of your mouth stays uncovered, so food tastes as it should

Downsides to Consider

All-on-4 is a major treatment, and it is not right for everyone. The main things to weigh up are:

  • Less back-up: with four implants, if one fails, the bridge may need to be remade on a new implant
  • Cleaning: fixed bridges need daily cleaning with interdental brushes or a water flosser, and regular professional cleans
  • Adjustment: speech and eating can feel different for the first few weeks
  • Repairs: the bridge can wear or chip over time and may need repairing or replacing
  • Cost: it costs more upfront than dentures

All-on-4 Cost in London

All-on-4 at Marylebone Smile Clinic starts from £12,000 per jaw, and from £20,000 for both jaws. Our full arch fees are all-inclusive, covering your consultation and 3D scan, extractions, temporary teeth while you heal, your final bridge and review visits. Your exact fee is confirmed once we have your CBCT scan, and set out in a written treatment plan before any treatment starts.

In the UK, All-on-4 is advertised from under £6,000 to over £15,000 per jaw. Lower prices often exclude the temporary teeth, extractions or a zirconia final bridge. Ask what material the final bridge is made from, how many appointments are included and what the guarantee covers. Treatment can be spread with 0% finance over up to 24 months, or over up to 60 months with interest, subject to status.

The All-on-4 Process

1. Consultation and 3D Scan

We check your teeth, gums, bite and smile line, and take a CBCT scan to plan the exact position and angle of each implant.

2. Surgery Day

Any remaining teeth that need to come out are removed and the four implants are placed under local anaesthetic. IV sedation is available if you are nervous. It keeps you awake but deeply relaxed, and is provided by experienced sedationists including Dr Damien Reilly, a Consultant in Special Care Dentistry. Where it is suitable, fixed temporary teeth are fitted on the day of surgery. If an immediate temporary is not suitable for you, we make a temporary denture for you to wear while the implants heal.

3. Healing

The implants bond with the bone over around 12 weeks. You eat soft foods for the first few weeks and attend review visits.

4. Final Bridge

Around 3 to 4 months after surgery, your final bridge is fitted, with its shape and shade agreed with you first. Your final bridge is made from a titanium bar with zirconia over it. It is screwed to the implants, so it does not come out at home, but we can remove it for professional cleaning.

Frequently Asked Questions

What is the average price of All-on-4 dental implants in the UK?

All-on-4 dental implants in the UK are typically advertised from under £6,000 to over £15,000 per jaw. The price depends on the bridge material and what the quote includes. At Marylebone Smile Clinic, All-on-4 starts from £12,000 per jaw and from £20,000 for both jaws.

How many teeth do you get with All-on-4?

An All-on-4 bridge usually has 10 to 12 teeth per jaw. Most people do not need back molars replaced to chew comfortably, and fewer teeth at the back keeps the bridge strong.

What are the downsides of All-on-4 dental implants?

The main downsides of All-on-4 are that there is less back-up if one implant fails, the bridge needs careful daily cleaning, it can take a few weeks to adjust to speaking and eating, and the bridge may need repairs over time. It also costs more upfront than dentures.

How long do All-on-4 implants last?

With good cleaning and regular hygiene visits, All-on-4 implants can last for many years. The bridge itself may need repairing or replacing over time, depending on the material and how you bite.

Is All-on-4 painful?

All-on-4 surgery is carried out under local anaesthetic, with IV sedation available, so you should not feel pain during the procedure. Swelling and discomfort for a few days afterwards are normal and are usually controlled with over-the-counter painkillers.

Can I have All-on-4 on just one jaw?

Yes, All-on-4 can be used on just the upper or just the lower jaw. Many people have one jaw treated first and the other later, or keep their natural teeth in the other jaw.

Am I too old for All-on-4?

There is no upper age limit for All-on-4. What matters is your general health and the bone you have. Many people have All-on-4 in their 60s, 70s and older.
